Erectile dysfunction, often called ED or weak erection, is when a man regularly cannot get or keep an erection firm enough for sex. It is very common, especially after the age of 40, but many men suffer in silence or turn to unregulated “power” products. Understanding the causes of erectile dysfunction is the first step to effective treatment. In many cases, ED is also an early warning sign of other health problems that need attention.

How an erection works

An erection depends on several systems working together. Sexual desire sends signals from the brain through the nerves. These signals relax the muscles in the walls of the blood vessels in the penis. Blood flows in and fills spongy tissue, making the penis firm. At the same time, the veins that drain blood are squeezed, keeping it in place.

Anything that affects the brain, the nerves, the hormones or the blood vessels can interfere with this process. That is why the causes of erectile dysfunction are so varied.

Finding which step is affected guides treatment, from lifestyle change to tablets or counselling.

Occasional problems are normal

Almost every man has trouble with an erection from time to time, for example when tired, stressed or after drinking. This is not erectile dysfunction. ED means the problem happens often, over several weeks or months, and affects your sex life or relationship.

Physical causes of erectile dysfunction

In most men over 40, physical causes are the main factor. The most common include:

  • Narrowed blood vessels. The same process that blocks heart arteries also affects the small arteries of the penis.
  • High blood pressure. It damages blood vessel walls over time.
  • Diabetes. High blood sugar damages both nerves and blood vessels. Men with diabetes often develop ED earlier.
  • High cholesterol. It contributes to narrowing of the arteries.
  • Obesity and lack of exercise. These raise the risk of all the conditions above.
  • Hormone problems, including low testosterone, thyroid disease and high prolactin.
  • Nerve damage from spinal injury, stroke or multiple sclerosis.
  • Surgery or radiotherapy for prostate, bladder or bowel cancer.
  • Peyronie’s disease, which causes scar tissue and a curved penis.

Erectile dysfunction as a warning sign

The arteries in the penis are smaller than those in the heart. As a result, they often show damage first. Erectile dysfunction can appear several years before a heart attack or stroke. Any man with ED should have his blood pressure, blood sugar and cholesterol checked. Treating these conditions protects both your erections and your heart.

Medicines that can contribute

Some medicines list ED as a side effect. Examples include:

  • Some blood pressure medicines, such as certain water tablets and beta blockers
  • Antidepressants, especially those in the SSRI group
  • Antipsychotic medicines
  • Some medicines for an enlarged prostate
  • Hormone treatments for prostate cancer

Never stop a prescribed medicine on your own. Talk to your doctor or pharmacist, because alternatives are often available.

Lifestyle causes

Lifestyle plays a big role in the causes of erectile dysfunction:

  • Smoking damages blood vessels and greatly raises the risk
  • Heavy alcohol use affects nerves and hormones
  • Cannabis and other recreational drugs can interfere with erections
  • Poor sleep and untreated sleep apnoea lower testosterone
  • Long-distance cycling on a hard saddle can press on nerves and blood vessels

Psychological causes

The mind is a powerful part of sexual function. Psychological causes include:

  • Stress about work, money or family
  • Anxiety, including performance anxiety after one bad experience
  • Depression
  • Relationship problems or poor communication
  • Guilt, fear or past trauma related to sex

Psychological causes are more common in younger men. A useful clue is that men with mainly psychological ED usually still have normal morning erections or can get an erection when alone.

How doctors find the cause

A doctor will ask about your symptoms, health, medicines and lifestyle, and examine you. Blood tests usually include blood sugar or HbA1c, cholesterol and, if needed, testosterone, ideally taken in the morning. Your blood pressure and weight will be checked. These simple tests often reveal treatable causes of erectile dysfunction.

Questions your doctor may ask

To find the causes of erectile dysfunction in your case, your doctor may ask:

  • When did the problem start, and did it come on suddenly or slowly?
  • Do you still have erections in the morning or at night?
  • Is the problem with getting an erection, keeping it or both?
  • Do you have problems with desire or ejaculation?
  • Which medicines, supplements and herbal products do you take?
  • How much do you smoke and drink?
  • How are things in your relationship and at work?

Honest answers help to find the right treatment quickly. Everything you say is confidential.

Treatment options

Treatment depends on the cause, but the most common options are:

  • Lifestyle changes. Losing weight, exercising, stopping smoking and cutting down on alcohol can improve erections, sometimes without other treatment.
  • Tablets. Medicines such as sildenafil  1  and tadalafil  2  increase blood flow to the penis when you are sexually aroused. They need a prescription and a medical check, because they are not safe for everyone.
  • Treating underlying conditions, such as diabetes, high blood pressure or low testosterone.
  • Talking therapy. Sex therapy or couples counselling helps when stress, anxiety or relationship problems are involved.
  • Other options, such as vacuum devices  3 , injections into the penis and, rarely, surgery, are used when tablets do not work.

HUBPHARM PHARMACIST’S NOTE
Genuine sildenafil and tadalafil are safe for most men, but a pharmacist must first check them against your heart and blood pressure medicines. Book a discreet chat with a HubPharm pharmacist. Orders are delivered in plain packaging.

Using ED tablets safely

Sildenafil, tadalafil and similar medicines must never be taken with nitrate medicines for chest pain, such as glyceryl trinitrate. The combination can cause a dangerous drop in blood pressure. Common side effects include headache, flushing, a blocked nose and indigestion. If an erection lasts more than four hours, go to hospital immediately, because this can damage the penis.

The danger of “power” products

Herbal mixtures, roots, capsules and drinks sold for sexual power are widely available in markets, motor parks and online. Many are unregulated. Some contain hidden prescription medicines, sometimes at high or unknown doses. These can cause dangerously low blood pressure, especially in men with heart disease, and they do nothing to treat the real causes of erectile dysfunction.

Erectile dysfunction in younger men

ED is not only a problem of older age. Younger men are more likely to have psychological causes, such as performance anxiety, stress or relationship pressure. Heavy alcohol, cannabis and anabolic steroids used for bodybuilding are also common causes of erectile dysfunction in young men. Pornography use is sometimes linked to difficulties with partners, although research is still developing. A young man with ED should still have a medical check, because diabetes and high blood pressure can begin early.

How partners can help

ED affects both partners. A partner may wrongly feel unattractive or suspect infidelity. Open, calm conversation helps to remove blame. Focusing on closeness and touch, rather than only on intercourse, reduces pressure. Attending appointments together shows support, and couples counselling can help when the causes of erectile dysfunction include relationship stress.

Talking about it

Many men feel embarrassed to raise ED with a doctor or partner. However, doctors and pharmacists discuss it every day, and it is nothing to be ashamed of. Talking openly with your partner reduces pressure and helps you find solutions together. Read more about why intimacy matters for health in our article on the benefits of a good sex life.

Getting help

Understanding the causes of erectile dysfunction opens the door to treatment that works. The NHS guide to erection problems has more detail. For a discreet consultation about genuine ED medicines, checks on interactions with your heart or blood pressure tablets, or advice on lifestyle changes, message a HubPharm pharmacist or ask Afiya on WhatsApp.
